Form Setup
Set the action attribute on your form to forms/mailchimp.php. This points your form submissions to the provided MailChimp subscription handler.
The form inputs in your form should match the fields accepted by your MailChimp List.
You may also like to include a required checkbox which offers a disclaimer about your privacy policy and how the user's data is handled.
